To nominate a child please fill out the form below COMPLETELY. Incomplete forms will NOT be considered.

Please read all of the rules and regulations before filling out the form below:

• Nominations are open to children ages 3-10 years old.
• Limited amount of bikes available. 
• Nominating a child does not guarantee they will receive a bike.
• Bike pick-up day (Distribution Day) is December 20th. This will be the only time bikes will be available for pick-up. If bikes aren't picked up on December 20th they will be donated to local charities.
• If the child you are nominating received a bike in the past they are unable to receive a bike this year.
• The deadline to apply is December 8, 2025
• If your child is chosen to receive a bike you will be notified by phone by or on December 15th.

Letters may be read on air on Hot Z95, Rock 92/7, and 1440 KEYS.
